do u have wide slider mod installed? maybe that is the reason

Nah don't uninstall Wide Slider. You need it.

Your issue seems to be unrelated to Wide Slider. It looks to me like just the eyebrows and the eyes that are not loading properly (are you getting the eye patch to load?). My best guess would be that some other mods that you have installed are conflicting with one another. Don't quote me on that though.
Maybe try uninstalling a few to see if that helps?

i do get the eye patch to load, but I hid it just to get a better picture
I'll try to figure out what mod is causing this issue

hs face project, it might be the reason since most of the card usually use/have this mod installed because it have wider selection of face shape and type

Go to the eye settings and lower the values that are over 100 till you find the problem areas; in earlier versions of wideslider, some attributes showed no difference between 100 and 200, so it was easy to drag the slider past the intended value.
