This is a CHARACTER CARD. It's a PNG which also contains character data just larger than usual.
You need;
1- GGmod
2- AC highheels
3- Wideslider mod
4- The hair is optional, it's Nier commander hair. Pretty sure it's from a hairpack from pastebin heaven.
5- YOU ABSOLUTELY NEED TO edit a text file.Otherwise you'll get feet and hooves both and all kinds of oddities.

FAQ
A- GGmod and AC high heels mods must be installed before you extract the zip, if you done it wrong extract the zip again.
B- If you get feet and hooves together you've missed the 3rd step. This can also happen due to a bug within GGmod itself, just swap between outfits and it'll be fixed.
B- If you don't have GGmod AND AC highheels this will never work.

Done every step, made sure again and again, still get both feet and hooves.

^ Responding to my previous comment, wearing most of the AC High Heels fixes this, the card didn't have any shoes on by default, might be a thing with mod IDs.

OP here, I likely moved them using MoreslotID & conflict resolver.
If you get both feet and hooves, change shoes to AC invisible heels as you're supposed to get them with the outfits.
